Once you apply and are approved for one program, you will be able to log into your student page to apply for the second program (when applicable)

If you already have records in the Outcomes Student Information System, please log in via the student page and complete the form there.

If you have no existing records or do not know, you can add all needed information to this form and submit when ready.

Identifying Information

* Status Card # - also known as INAC or Treaty Number - is a 10 digit number made up of your band number, family number and position number.

Note, on one application you can only apply for semesters from one fiscal year. To include semesters from multiple fiscal years, additional applications must be filled out.

Final Application Details

Please select your Priority Level based on the following guidelines.

Priority 1a
Continuing students (including those who have self-funded).

Priority 1b
Newly graduated students who have obtained their high school diploma and are moving on to post secondary studies, or those who have obtained the equivalent (i.e., GED) and are moving on to post secondary studies.

Priority 1c
Students who have never attended a post-secondary institution.

Priority 1d
Reapplying students who have previously withdrawn from their program for certified medical reasons shall be entitled to reapply within two years of withdrawing from their program of study. At the end of the two years, a medical note and/or other valid documentation to support the return of the students must be included in the post secondary application form.

Priority 1e
Deferred students (eligible but not funded in the past due to limited funding availability).

Priority 2a
Students who are entering post-secondary studies as a mature student. Prior Learning Assessment Review (PLAR) documentation is to be provided with your application by the funding deadline date.

Priority 3a
Students who were required to discontinue by a post secondary institution, but have now been readmitted.

Priority 3b
Students who have overpayments with no payment arrangements on file.
